What a Composite Cardiovascular Endpoint Means
A composite endpoint combines several defined events into a single outcome.
Dr. Sherry-Ann Brown, MD, PhD, FACC, FAHA
The research question
A composite endpoint combines several defined events into a single outcome. It can make a cardiovascular trial more feasible by collecting clinically important events under one framework. Interpretation begins by naming every component and understanding how each is identified. Readers should also ask whether components have comparable importance for patients.
How to interpret the evidence
A total composite result can be influenced more strongly by the frequent components. Component analyses help explain the pattern, although they may have fewer events and wider uncertainty. Careful communication states the composite finding first, then describes individual components according to the study analysis. Patients can then connect the research outcome to the experiences they value most.
The SELECT primary cardiovascular composite included cardiovascular death, nonfatal myocardial infarction, and nonfatal stroke.
A composite can also include events of different severity. The educational explanation should preserve this clinical distinction, especially when patients value avoiding some outcomes more strongly than others. Component frequency, effect estimates, and uncertainty help readers understand the combined result. A concise composite label alone may leave those meaningful details invisible.
Practice the appraisal
Original endpoint exercise: construct a component table with cardiovascular death, myocardial infarction, and stroke as rows. For a real trial, record the definition, ascertainment process, and reported event information for each. Then ask what happens to the composite interpretation when one component is substantially more frequent. A hypothetical composite could improve through that common component while estimates for rare components remain imprecise. Present this as an illustration of interpretation rather than a claim about SELECT. The exercise encourages readers to inspect the whole composite and the clinical meaning of its parts.
Apply the lesson
In a patient discussion, spell out the composite components and connect them to recognizable events. Explain the overall finding using the study population and follow-up period. Component results can provide useful detail when their uncertainty is clear. A team preparing education should check that a composite benefit has not been rewritten as a precise benefit for every individual component.
